DOH-Hernando’s Family Health Clinic is now accepting Medicaid patients.

The Family Health Clinic is a primary care clinic for insured and uninsured clients.

Routine medical services are offered on a sliding fee scale to adults and children who do not qualify for insurance. Fees are based on the declared income. No one will be denied access to services due to inability to pay. There is a discounted/sliding fee schedule available based on family size and income.

The following Primary Care services are provided:

  • Primary Care
  • Lab work and necessary tests
  • Prescriptions
  • Cancer screenings
  • Specialty care
  • Behavior health screening and referral for treatment
  • Wellness and prevention services, including annual physicals
  • Management of chronic diseases, such as asthma, high blood pressure, and type 2 diabetes
  • Epilepsy and Diabetic Medication assistance programs
  • Case management services
  • Referral for dental services
  • School physicals 
  • Immunizations for children 0-18 years of age
  • Annual check-up/preventative care for adults and children
  • Access to volunteer services

Immunizations and STD Testing and Treatment services are also offered, no insurance required.

The Family Health Clinic is open Monday through Friday from 8 a.m. to 5 p.m.
