Immunizations are available on the 2nd and 4th Mondays and every Monday through Wednesday by appointment, 352-540-6800 at our Brooksville office, 300 South Main St.

Immunization Record Request
If you would like to request a copy of your child’s DOH-Hernando immunization record, the complete name, date of birth, and social security number is required. If no immunizations are administered when requesting the child’s record, there is a $10.00 fee.
Parents with proper identification can pick up a copy of the immunization record from any county health department (CHD) in the state of Florida. All CHDs are part of the state immunization registry FLShots.
